Contracted Insurance Plans
Cornerstone Pediatrics is considered an In-Network
provider by most insurance plans. If you are unsure as to whether
or not we are an In-Network provider under your plan, please contact
your insurance company or employer group.
Cornerstone Pediatrics is currently not accepting
new families to our practice who are covered under the following insurance
plans:
• Medicaid
• CareSource
• Molina
• Amerigroup
If your child has one of these insurance plans
and is an established patient or a sibling of an established patient,
we will be happy to provide care for your child. Please notify our
receptionist of this relationship when you call to make your appointment.
Filing of Insurance Claims
Cornerstone Pediatrics has agreed to file claims
to insurance for patients who have coverage. In order to do this as
accurately as possible, you must have your child’s insurance
card available at each visit, and if you participate with a managed
care program, one of our physicians’ names should appear on
the card.
Verification of Coverage
We are unable to verify coverage by telephone
or internet when you come in for a visit. It is the parent’s
responsibility to have this information available to whoever is bringing
the child for a visit (spouse, grandparent, nanny, etc.).
Billing Procedure
A monthly statement will be sent to you detailing
balances that are due from you. The balance shown on your statement
is due immediately upon receipt. If you have questions regarding your
bill, or if you are working with your insurance company to get payment
on a denied claim, we ask that you contact our Billing Department
at (937) 886-3878 as soon as possible in order for your child’s
account to remain in good standing.
Self Pay Patients
If your child does not have any insurance coverage,
you may contact our Billing Department for an estimate of the charge
for a particular visit, which will include a 15% self pay discount.
Payment in full is due at the time of service.
Copay Collection
Your child’s copay is due at the time of
service, regardless of who brings the child to the appointment. Cornerstone
Pediatrics reserves the right to charge a $20.00 fee for any co pays
that are not paid at the time of service.
Separated and Divorced Families
For those families where parents are separated
or divorced, the parent authorizing treatment and bringing the child
to be seen is responsible to us for payment. All payments are due
in full at the time of service.
If a divorce decree requires the other parent
to pay all or part of the treatment costs, it is the authorizing parent’s
responsibility to collect from the other parent. Cornerstone Pediatrics
cannot act as a mediator for collection of payment.
A copy of the bill with appropriate insurance
coding will be given to the authorizing parent at each visit. If additional
copies of the bill are needed, please ask the receptionist before
leaving the office.
Inability to Pay
If you have extenuating circumstances that will
prevent you from paying your copay or any balance due at the time
of service, this should be discussed with the Billing Department prior
to your appointment.
Collections Procedure
If satisfactory arrangements cannot be made in
order to resolve your child's account in a timely manner, the patient
and his/her siblings may be considered for discharge from our practice
and the account guarantor's information submitted to a collection
agency. In order to avoid collections proceedings, please respond
to each monthly bill by either remitting payment in full or speaking
with our Billing Department.
